Job Overview

Scion Hospital is seeking a qualified and experienced Health Records Information Officer to manage patient health information, clinical data, medical records, and health information systems.

The successful candidate will support accurate medical record management, clinical coding, health data analysis, statutory reporting, data privacy, and the effective operation of hospital information systems.

About the Role

The Health Records & Information Officer will be responsible for ensuring that patient records are accurately captured, securely maintained, easily retrievable, and compliant with applicable health information and data protection requirements.

The role also involves clinical coding, health statistics reporting, electronic health records management, and conducting regular audits to improve the quality and completeness of medical records.

Key Responsibilities

Patient Registration & Records Management

Manage patient registration and maintain the Master Patient Index (MPI).

Maintain accurate physical and electronic patient health records.

Apply standardized medical record filing systems, including Terminal Digit Filing.

Ensure patient records are properly archived and easily retrievable.

Maintain confidentiality and security of patient information.

Clinical Coding & Indexing

Assign accurate disease and procedure codes using ICD-10/ICD-11 guidelines.

Support clinical audits and insurance billing processes.

Assist with morbidity and mortality classification.

Ensure accurate indexing and coding of clinical information.

Health Data Analysis & Reporting

Compile and validate routine hospital performance statistics.

Prepare health information reports, including indicators such as:

Bed Occupancy Rate (BOR).

Average Length of Stay (ALOS).

Bed turnover.

Support timely monthly statutory reporting.

Upload required health data to national platforms such as DHIS2.

Analyze health information to support hospital administration and quality patient care.

Data Privacy & Compliance

Maintain strict patient confidentiality.

Ensure health information is securely handled and protected.

Comply with applicable data protection legislation and institutional policies.

Implement appropriate procedures for protecting sensitive patient information.

Hospital Information Systems

Administer daily Hospital Information System (HIS) and Electronic Health Record (EHR) workflows.

Maintain data integrity within health information systems.

Resolve user queries relating to health information systems.

Support downtime procedures during system outages.

Ensure accurate and timely data entry across hospital systems.

Records Quality & Auditing

Conduct regular quantitative and qualitative health records audits.

Identify incomplete or inaccurate documentation.

Support improvements in clinical documentation quality.

Ensure health records meet required standards and institutional procedures.

Qualifications

Diploma or Bachelor's Degree in Health Records & Information Management from a recognized institution.

Valid licence and active registration with the relevant Health Records Officers professional board/body.

Minimum 2–3 years of experience working in a busy hospital environment.

Experience working with Hospital Information Systems (HIS) and Electronic Health Records (EHR).
